If you disable promotion and relegation then nobody will move up or down the tiers, provided their are enough players in each tier to allow them to promote/relegate.

You can only enable or disable relegation. Relegation is automatically disabled if the tier is less than half full, however.
In 2 car leagues, the best 2 managers get promoted, and the 2 at the bottom of the standings are demoted.
In 1 car leagues, the best 4 managers get promoted, and the 4 at the bottom of the standings are demoted.
